Service User Agreement

Eberwein Medical & Lifestyle takes patient safety and data security seriously. To prevent impersonation and ensure appropriate age we require that all patients verify their age and identity prior to appointments. The can be in the form of a photograph or scanned image of a passport, driving licence or identity card. Arrangements for secure upload will be made.

When using out service only use your own smartphone, tablet or computer. Do not use any shared devices. Ensure your devices are up to date with the latest Android, iOS, Windows or Mac updates. If using a computer please ensure that you have an up to date virus scanner installed.

All appointments should take place in a private place and not in a coffee shop, library or somewhere that is open to the public. If you live with other people in your household please ensure that you would not be interrupted by them. If you consent to them being present this is not a problem but please be mindful of the potentially personal nature of the consultation. You current location during the consultation will be asked for. This is in the advent of an emergency so that the emergency services can be called if needed.

All communication with Eberwein Medical & Lifestyle is subject to strict confidentiality in accordance with national legal frameworks. Full GDPR is strictly adhered to. Only contact Eberwein Medical & Lifestyle via the secure forms on our website or our encrypted email. Any clinical communication from Eberwein Medical and Lifestyle will be via encrypted video or password protected email messages. All data is stored on secure servers. At present we are only treating adults aged 18 and above. All Service Users are requested to give consent to receiving care and treatment from Eberwein Medical Ltd based upon the principles and understandings in our General Consent and Welcome Document. A secure electronic version of the consent form will be emailed to patients for digital signature.

It is our wish to provide the highest standards possible for you. We want to ensure that you are treated safety and give you the best care, both with us now and with anyone else in the future. In order for this to happen it is paramount to get an accurate past medical history. For this reason, in some cases we we ask you to provide a Summary Care Record and a list of recent blood results. This can easily be obtained by asking your GP receptionists. We will also ask for consent to keep your GP updated with your treatment with us.
